Clinical NursingPractice Management & OperationalExcellence
- Assist indeveloping community nursing guidelines and protocols to guideclinical and professional practices
- Monitorthe community nursing team's practice on resource management andrecommend strategies to reduce waste in service delivery, care andtreatment
- Ensure the appropriateness andcost-effectiveness of practices, equipment and products used forthe clients
- Participate in quality audits,quality improvement and evidence-based projects
- Identify and report potential threats to the practice ofethical and legal principles in community nursing servicedelivery
- Conduct risk assessment to identifyrisks and safety hazards of community nursing practice andimplement measures to mitigate risks identified
- Execute responsibilities as per emergency protocols inthe event of public health threat oremergency
- Participate in the development and implementation of aperson-centred care (PCC) delivery model.
- Support cultivating organisational culture and aligningstrategies, goals and framework to enable the delivery ofPCC.
- Facilitate interdisciplinary teamdiscussions to align and prioritise care goals
- Evaluate PCC plans, incorporating anticipatory careneeds, in consultation with the interdisciplinary team
- Manage residents with complex care needs in collaborationwith the interdisciplinary care team and the larger community caresystems
- Perform medication managementincluding administration, titration, reconciliation and educationin accordance with organisational guidelines andprotocols
- Recognise early signs ofdeterioration in residents, intervene and escalateappropriately
- Engage residents, families andcaregivers for Advance Care Planning/Preferred Plan of Care incollaboration with primary care teams
- Ensurethat timely and accurate documentation is performed by the careteam as per organisational standards andguidelines
- Be grounded to provide ongoing clinical supervision tocare staff and work with them to ensure safe and qualitycare.
- Conduct incidental and bedside teachingto enhance their competencies when rendering care to theresidents.
- Follow-up with care staff posttraining they have attended and assess their competencies using thecompetencychecklist
- Advocatefor residents', families'/sponsors', caregivers' and/or thecommunity's needs and their readiness for community careinterventions
- Build therapeutic relationshipsand professional boundaries when dealing with residents,families/sponsors, caregivers and/or community partners
- Evaluate effectiveness of resident, family/sponsor and/orcaregiver training and education and recommend follow-upactions
- Collaborate with care team members andcommunity partners to recommend appropriate interventions forresidents
- Assist in the implementation of systems and processes toensure compliance with MOH and regulatory standards
- Work closely with SNM QM and SCM/NM to plan and preparefor inspection and renewal of licences for all nursing homeservices
- Participate in developing of the riskmanagement strategies, including identification, monitoring andevaluation of nursing clinical quality indicators.
- Participate in managing emergency response and crisismanagement plans in accordance with organisational directionincluding collaboration with relevant departments andstakeholder
- Perform any other roles andresponsibilities as assigned bySCM
- Possess a minimum of a Bachelor's Degree inNursing
- State Registered Nurse with validpracticing license issued by the Singapore Nursing Board
- Valid certification in Basic Cardiac Life Support(BCLS)
- At least 4-6 years of clinicalexperience as SSN, preferably in the community care sector will bean added advantag
- Clinical supervision andteaching skills
- IT savvy - MSWords/Excel/Powerpoint
- Strong communicationsand inter-personal relationship skills
