The NUHS RHSO Primary Care Partnerships is a division underthe NUHS Regional Health System Office (RHSO) that focuses onmanaging the transition of patients with well-controlled chronicconditions from Specialist Outpatient Clinics (SOCs) to primarycare partners in the community. This initiative aims to decongestacute hospital services and provide quicker access to acute careresources. The key primary care partners include St Luke'sOutpatient Clinic, Family Medicine Clinics (FMCs), NationalUniversity Polyclinics (NUPs), and private GPs. The team of carecoordinators plays a crucial role in ensuring a smooth transitionfor patients by addressing care and cost concerns, providingdetailed counselling, and offering value-added services such asappointment consolidation and GP empanelment on a needsbasis.

As Assistant / Care Coordinator (PrimaryCare Partnerships) in RHS Office, you are responsible for thefollowing areas:

JobResponsibilities
  • Perform patient selection, patient counselling,appointment consolidation and referrals from NUHS hospitals to NUHSprimary care partners
  • Provide training anddevelop workflows for hospital departments to support them in theright-siting efforts
  • Support the continuingcare of patients in the community
  • Facilitatethe transition of patients from polyclinic to NUHS GPpartners
  • Enter data into database fortracking
  • Engage patients for health promotionactivities and participation in NUHS or national initiatives as andwhen required
  • Perform any duties as assignedby supervisors andHOD
JobRequirements
  • Possess aDiploma / Degree (Preferably in Health or Socialrelated)
  • At least 1 - 2 years of relevant workexperience (Fresh Graduate are welcome to apply)
  • Able to multi-task and work independently and as ateam
  • Strong organisational, coordination,interpersonal and communication skills
  • Able torespond to common inquiries or complaints from doctors, staff andpatients
  • Proficient in MicrosoftOffice
  • Able to converse in local languages anddialects is preferred
