  • ServiceCare Coordination and Case Management.
  • Employseffective communication and rapport building skills wheninteracting with antenatal patients, clients, families andstaff.
  • Collaborates with interdepartmental andexternal colleagues with the aim of providing holistic care andoptimise health outcomes for antenatal clients recruited under theHome Visit Programme.
  • Administrative andSystems Support. Support team with outreach and engagement effortsas required by programme.
  • Assist inappointment scheduling and rescheduling and addresses clientenquiries. (e.g. teaches the use of the Health Buddy for assessingtheir records and services (e.g. changing appointments).
  • Navigates, troubleshoots and initiate discussions for ITissues that impacts service and operations, e.g.OAS/SAP/SCM/SSNet.
  • Identifying needs andscreening. Effectively manage patient caseloads at clinictouchpoints.
  • Performs health and socialscreenings for antenatal clients. (E.g. nutritional screening,mental health screening, home readiness and safety etc.)
  • Maintains documentation and complete reportingrequirements according to organisation standards.
  • Performs other job-related duties as assigned by ProgramLead, Social Service Manager or TrainingManager.


Requirements:
  • Diploma with 2 - 4 years' relevant experience, or GCEN/O/A Level, or NITEC in Healthcare/Equivalent with 6 - 10 years'relevant experience
  • Ability to communicateeffectively, both orally and in writing
  • Possesses records maintenance as well as organizing andcoordinating skills
  • Strong word processing,receptionist and data entry skills
  • Hasknowledge of supplies, equipment and/or services ordering andinventory control
  • Ability to maintaincalendars and schedule appointments
  • Ability tounderstand and follow specific instructions andprocedures
  • Skilful in the use of operatingbasic office equipment
  • Ability to dealamicably with all levels of staff and external customers
  • Ability to maintain confidentiality of records andinformation
  • Supervisoryskills
