Roles & Responsibilities

Responsibilities:

  • Perform medication reconciliation & review, and ensure the appropriateness of patient drug therapy.
  • Dispense medications and give appropriate counselling to patients and caregivers.
  • Provide timely and accurate drug information to healthcare professionals and patients / caregivers.
  • Be involved in educating and training of pharmacy staff and other healthcare professionals.
  • Participate in projects/activities which support the goals of the department and hospital.

Requirements:

  • Degree in Pharmacy.
  • Registered Pharmacist with the Singapore Pharmacy Council.
  • Minimum 2 years of experience and demonstrated competency, or with relevant post graduate qualifications.
  • Have experience in clinical patient care setting.
  • Strong analytical skills.
