Roles & Responsibilities

JOB SUMMARY

Provide support and care for residential and day care members in their journey towards recovery, community re-integration and an improved quality of life.


KEY DUTIES AND REPONSIBILITIES

  1. Caring and providing support for 30 residential / daycare cases.
  2. Providing rehabilitative counselling / initiating referrals / organizing family sessions / making home visits for residential / daycare cases when required.
  3. Conducting rehabilitative programmes such as skills training and mental wellness activities.
  4. Providing weekly follow-up on cases and reviewing them with the Review and Admissions Committee periodically.
  5. Interviewing and making assessments on individuals / families who seek admission to the Centre.
  6. Consulting and discussing with the Manager when cases involve disciplinary measures and when encountering any difficulties.
  7. Exploring and implementing transition / discharge planning for cases.
  8. Attending Review & Admissions Committee Meetings, Staff Meetings, Case Consultations, Case Conferences, etc.
  9. Maintaining documentations and records for services provided to cases.
  10. Assisting in other duties and functions assigned by the Manager and/or Centre Director.


JOB REQUIREMENTS

  • Degree in Counselling, Psychology, Social Work or related field
  • At least 2 years of relevant experience
  • Passion to work with persons with mental health issues
  • Patience and compassion, microskills in listening
  • Knowledge in psychiatric rehabilitation and case management skills
  • Good interpersonal and communication skills
  • Team player
