Roles & ResponsibilitiesPosition Overview:
The individual will case manage ex-offenders who come to ISCOS to seek assistance. The individual will evaluate the needs of client, identify the services needed, and create a plan to provide that care. Care & Advocacy Specialist may also be responsible for evaluating the effectiveness of the care plan and services.
Principal Duties and Responsibilities:
- Do case intake and identify client's their needs, strengths, and barriers to successful reintegration.
- Develop individualized reintegration plans for clients, outlining steps to address housing, employment, education, healthcare, substance abuse treatment, and other pertinent needs.
- Provide support and resources for clients; build good working relationship with support networks, government resources, and community resources.
- Provide information and refer clients to internal programmes and services under ISCOS or coordinate with external agencies, service providers, and community organizations to facilitate access to essential resources and support services.
- Collaborate with clients to establish realistic short-term and long-term goals tailored to their specific circumstances and aspirations.
- Collecting and reviewing documents of clients.
- Supports and monitors clients' progress in improving their resources and ability to cope with their challenges.
- Participate in case discussions with external agencies.
- Participate and provide updates of cases during case conference with Manager, Programmes and Services, Deputy Director and Executive Director.
- Conduct regular home visits, phone calls and message clients to check-in on them.
- Maintain accurate and up-to-date records of client interactions, service plans, progress notes, and outcomes in compliance with organizational policies and regulatory requirements.
- Prepare regular reports documenting program activities, client outcomes, and challenges encountered, for internal use and funding purposes.
- To facilitate Men support group.
- Apart from the above, the officer has to perform any other duties as assigned by his superiors for the purpose of fulfilling organisational needs.
Supervision Received:
Supervision is provided by the reporting supervisor. Position requires ability to perform with minimal supervision.
Supervision Exercised:
No direct reports. May monitor and coordinate the admin work of temps.
Core Competencies:
- Proficient in MS Office.
- Good team player and have strong problem solving skills, communication and interpersonal skills.
- Good report writing skills.
- Ability to work with tight deadline and is strong with follow up.
- Excellent organizational and time-management skills, including ability to handle multiple tasks simultaneously and juggle priorities.
- Good knowledge about the community resources.
- Ability to work under pressure.
