Primary Care Coordinator

We invite highly motivated and dedicated Primary Care Coordinators to join our PCN team. As a Primary Care Coordinator, you play a vital role in facilitating effective coordination and delivery of primary care services within our Primary Care Network (PCN). You will work closely with healthcare providers, patients, and other stakeholders to ensure seamless care coordination and enhanced patient outcomes.
Responsibilities
Patient Care and Care Coordination
- Offer guidance and assistance to patients in accessing and navigating support services in the community as well as information on community programmes, services, available subsidies and schemes
- Coordinate and facilitate access to ancillary services and community programmes for patients based on care needs
- Support delivery of health plans to empower patients in self-management and assist patients and/or caregivers in using home monitoring devices
- Conduct and coordinate screening such as diabetic foot screening (DFS), diabetic retinal photography (DRP), and spirometry
Patient Education and Health Promotion
- Build rapport and trust with patients and caregivers and provide basic patient education on chronic diseases, e.g., lifestyle modifications, preventive health
- Offer information and guidance to patients on health promotion, screening, immunisation and assessing health educational resources
- Educate patients on recognising red flags or warning signs, empowering them to take timely action or seek medical help when necessary
Administrative Support
- Collate and organise referrals, reports and claims
- Participate in site visits to survey the clinic premises and ensure smooth operation of PCN services.
- Assist in conducting PCN briefings and with the onboarding process to newly enrolled clinics
- Utilize data analytics tools to analyse and interpret collated data and visual aids to enhance presentation delivery towards contributing to Quality Improvement Projects (QIPs)
- Ensure compliance of clinical requirements outlined by MOH
Communication and Team-Based Care
- Understand the role of individuals working within the PCN setting and work effectively within teams whilst ensuring own work is within professional scope of practice
- Establish rapport with GPs and clinic teams through effective communication and reliable follow-through on commitments, ancillary services and referrals
- Engage patients through active listening skills and shared agenda setting
- Communicate and document assessments and interventions clearly and appropriately
- Escalate when additional expertise or referral is necessary
- Support the implementation of the Healthier Singapore (Healthier SG) initiative, which aims to help Singaporeans and Permanent Residents take steps towards better health through a choice of their preferred family clinic, a customised health plan and access to community programmes
Qualifications
To be a PCN Primary Care Coordinator, you should possess the following requirements:
- Diploma or degree in any discipline, preferably in health sciences or related fields (Fresh graduates in related fields are welcome to apply)
- At least 2 years of relevant working experience in healthcare or social service settings (for non-fresh graduates)
- Good interpersonal and communication skills (verbal and written)
- Proficiency in Microsoft Office applications and basic IT skills
