Job Role Description
A Social Worker provides care, support, and interventions to students, their families, and partners in the wider community to address psychosocial needs in collaboration with various allied professionals. This involves providing access to financial assistance, performing community development work, and implementing programs that uplift the professional practice of Social Work.
S/He serves the needs of students and families across the different sectors within the Canossian Village and the wider community. S/He works collaboratively within a multi-disciplinary team, school staff, families, and relevant agencies to provide timely care and ongoing support to those in need. S/He conducts casework, group work, and implements programs to address service gaps, contributing to the Village’s efforts in providing an enhanced service delivery through the Canossian Circle of Care. S/He also participates in continuing education and research as needed.
S/He should be resourceful and possess strong collaboration, communication, and problem-solving skills to effectively work with multiple stakeholders.
Critical Work Functions and Key Tasks
Casework - Assessment and Intervention Planning
- Conducts intake, risk and needs assessments to evaluate and identify areas for intervention and follow up (e.g., referral to access internal and external community resources)
- Conducts means testing and assist parents with applications related to financial assistance, schemes or programmes offered in the community
- Conduct home visits as part of assessment or intervention follow-up
- Conduct care/ case planning and goal setting with family
- Prepare social reports
- Facilitate family’s understanding of needs of child and programmes that can be offered to better support the child and family
Casework - Delivery of Interventions
- Implement case and care plans and interventions for routine cases
- Participate in collaborations during team meetings or case conferences with relevant stakeholders (e.g., teachers, facilitators, parents, external community agencies) to ensure holistic case management for child and family for routine cases
- Review casework interventions for improvements
- Conduct closure of cases upon achieving sustainable service outcomes for clients, families and communities
- Utilise evidence-based practices for assessment, case formulation, intervention planning, implementation of intervention and progress evaluation
Casework – Performance of Clinical Administration and Operations
- Comply with profession's ethical guidelines and organisation's clinical protocols, and Standard Operating Procedures (SOPs)
- Manage feedback from stakeholders
- Complete documentations (e.g., intervention plans, progress reports, case notes, case updates to sectors) accurately and promptly, and ensure they are well-maintained
- Utilise resources in a cost-effective manner through managing individual caseload and completion of clinical and administrative work in a timely manner
- Work within budget allocated for the financial year
- Furnish one’s caseload, outputs and outcomes for reporting
Group Work
- Plan group interventions, facilitate group work under supervision, and review for improvements
- perform development and planning of programmes to address service gaps, implement and support evaluation of programmes
System Work – Within CCDU
- Identify areas of improvement in models of care and service delivery based on evidence-based practices
- Participate in research and evaluation for programme improvement
System Work – Within The Canossian Village
- Participate in sector or village-wide quality improvement projects/ programmes/ initiatives
- Assist in the planning and/or delivery of trainings/ workshops/ activities/ programmes to meet the learning needs for staff and caregivers
System Work – In the Community
- Conduct environmental scans and community need assessments
- Collaborate with other agencies to pool community resources
- Support development of and implement community devleopment initiatives
- Review community development initiatives for improvements
- Identify and tap on available community resources to support the families
Personal Development
- Enhances professional growth and development by keeping up to date with current literature and through participating in educational programmes, in-service meetings, and workshops (internal and external)
People Development
- Supervise and mentor Social Work interns under care, ensuring that they are growing professionally
General
- Performs any other duties as assigned by the Senior Social; Worker/Manager, CCDU
Education, Skills, and Competencies
Education and Experience
- Minimum Degree/Post Graduate Diploma in Social Work
- Registered or Accredited Social Worker
- Minimum 3 years of experience in the social service working with children and families
Technical Skills and Competencies
- Possess relevant knowledge and skills to perform the above job responsibilities with high quality standard
- Possess knowledge of community resources and service provision for children with special needs and their families within the Singapore context (From early years to primary years and across EI, mainstream and SPED settings)
- Able to advocate for clients and work to obtain needed resources to support client's to learn and develop their own systems.
- Able to take a series of actions and strategies to enable clients to achieve solutions
- Able to think clear and stay focused under pressure, adapts to harsh environment and thrives on continuous personal learning.
- Able to tailor communication of technical knowledge according to recipient's level of understanding
- Capacity to work effectively in response to work demands (responding to client needs in different circumstances including critical incidents)
- Able to provide guidance or professional assistance in response to requests from clients, other professionals, external organisations and the general public
