Social Worker - Transitional Shelter

Roles & Responsibilities
1. Referral**- Handle referral and seek clarification with referring agencies
- Conduct eligibility assessment for referral with referring agencies.
- Ensure timely submission of intake assessment onto system and uploading of required documentation.
2. Case Management**- Develop professional working relationship with fellow professionals, ministries, stakeholders and service users.
- Conduct needs/risk assessment for cases in shelter.
- Development risk management and intervention plans for cases.
- Engage and comanage complex cases with the relevant agencies through case review and/or case conference on a regular basis.
- Prepare and record all documentations stipulated by the organisation.
- Ensure data records are timely updated.
- Collect and organise necessary documents for reporting.
- Develop and execute programme for tenant in shelter.
- Conduct regular sessions and case plan review with tenants as per service requirements.
- Engage in case conferences, triaging and conversation with multidisciplinary team for complex cases,
3. Discharge Planning**- Plan discharge for tenant in the shelter
- Assist in the integration of tenants into their new environment.
- Link cases to relevant agencies for continuous social support.
- Work closely with stakeholders and community partners in the handover of cases.
_Technical Competencies:
_
- Degree in Social Work from an accredited institution.
- Accredited as Registered Social Worker with SASW.
- At least 3 years in direct social work is preferred.

- Strong interpersonal and communication skills.
- Understands the principles of engaging and working with vulnerable families and individuals.
- Case management and casework knowledge and skills.
- Risk assessment and risk management.
- Good report writing skills.
- Good conflict management skills
- Willing to work with the homeless
- To provide supervision for staff (senior level)
