JOB SUMMARY
To provide case management and counselling to members under psychiatric rehabilitation for recovery and re-integration into the community.

KEY DUTIES AND REPONSIBILITIES

  • Caring and providing support for 30 residential / daycare cases.
  • Providing rehabilitative counselling / initiating referrals / organizing family sessions / making home visits for residential / daycare cases when required.
  • Active cases must be seen on a weekly basis. For nonactive cases, each case must be reviewed at least once a month and the Review & Admissions Committee once in every three months.
  • Interviewing and making assessments on individuals / families who seek admission to the Centre.
  • Consulting and discussing with the Manager when cases involve disciplinary measures and when encountering any difficulties.
  • Consulting and discussing cases recommended for discharge with Review & Admissions Committee, exploring discharge plan and seeking their approval before discharge.
  • Attending Review & Admissions Committee Meetings, Staff Meetings, Case Consultations, Case Conferences, etc.
  • Maintaining documentations and records for services provided to cases.
Assisting in other duties and functions assigned by the Manager and/or Deputy Director/ Director.

REQUIREMENTS

  • Degree in Counselling, Psychology, Social Work or related field
  • At least 2 years of relevant experience
  • Passion to work with persons with mental health issues
  • Patience and compassion, microskills in listening
  • Knowledge in psychiatric rehabilitation and case management skills
  • Good interpersonal and communication skills Team player
