Overview

Reportingto the Pharmacy Section In-Charge, you will help provide safe,efficient and timely distribution (dispensing) of drugs in NUH. Youwill coordinate activities in your area of purview to be in linewith the objectives of the pharmacydepartment.

JobDescription

You will beresponsible for the following:

• Review writtenorder form (medication chart, medication record or prescription) todetermine the drug needs of the patient, ensuring that overdoseshave not been prescribed, that no toxic reactions will result fromadministration to the patient and that all drug orders are specific(contact physicians as necessary to review ambiguous or illegibleprescriptions)

• Fill prescriptions as orderedby attending physician by:

I. Filling bottles,packaging material, dosage form and other packaging forms withmeasured amounts of medication in accordance with prescriptionspecifications

II. Documenting supplies used andlabelling containers (including directions foruse)

• Supervise the compounding and/ordispensing of medication (unit dose, intravenous admixtures,investigational drugs and outpatientprescriptions)

• Explain directions forprescription use (drug therapy) to patients

•Monitor response of patient to prescribed drugtherapy

• Make recommendations about appropriatedrug therapy to doctors

• Carry out medicationreconciliation when appropriate

• Report adversedrug reactions encountered by patients

• Provideinformation to other health professionals

•Ensure patients receive their medicines correctly in a timelymanner

• Informs prescribers of any druginteraction, adverse drug reactions,contraindications

• Maintains a shelf inventoryof medications and pharmaceutical supplies, ensuring that storageis in accordance with security, environmental and product shelflife parameters

• Implement and adhere to thepolicies, practices and regulations established by the PharmacyDepartment, various NUH management committees and outsideregulatory agencies pertaining to pharmacy

•Perform inspections of drug storage areas throughout the hospitaland performs other job-related duties asassigned

• Treat each patient, visitor andemployee with compassion, courtesy and tact

•Assist in acquisition of special items for patients with approvalfrom senior pharmacist

• Any other duties orresponsibilities as assigned

JobRequirements

• Bachelor ofScience in Pharmacy from an accredited college by SingaporePharmacy Council.

• Registered with theSingapore Pharmacy Council

• Able to workindependently and in a team.

• Good listeningand communication skills.

• Possess basicsupervisory and management skills

• Flexibilityto work on weekends/public holidaysoccasionally.
