Caseworker | Geriatric

Roles & Responsibilities

• Develop individualise care plans for residents.
• Execute the care plan and monitor progress.
• Coordinate care for residents with the relevant stakeholders.

Job Requirements :

• Diploma/Bachelor’s Degree in Social Work, Psychology or Counselling.
• Preferably 1 year of relevant experience in Social Service sector or Healthcare.
• Willing to perform shift work in accordance with the duty roster, Mon to Fri (staggered office hours)
• 8 hours: 8am to 5pm / 8.30am to 5.30pm / 9am to 6pm
• 9 hours: 8am to 6pm / 8.30am to 6.30pm / 9am to 7pm
